Mental Health Facilitator (primary) Band 6

An exciting opportunity has arisen for an experienced Registered Mental Health Nurse (RMN) to join our Mental Health Facilitator Team. Working within GP surgeries across Leicester city, you will support primary care staff in delivering highquality support to patients diagnosed with a Serious Mental Illness (SMI). This includes helping to manage SMI registers in line with Quality Outcomes Framework (QOF) standards and supporting annual mental health and physical health reviews to achieve national NHS England targets.

The role involves assessing mental and physical health needs, promoting positive longterm outcomes, and providing shortterm, recoveryfocused support for individuals aged 16 and over. You will also contribute to service development, strengthen collaboration between primary and secondary care, and ensure effective signposting to relevant services. Flexibility, adaptability, and strong partnership working are essential. A full driving licence and access to your own vehicle are required.

The role will be based in GP Practices, the Community base and some homeworking.

You will be assigned to one of the Primary Care Networks (PCNs) across Leicester and will work autonomously within GP practices to provide mental health assessment, physical health checks, signposting, onward referrals, and followup appointments where necessary.

Ideal candidates will be Registered Mental Health Nurses (RMNs) with extensive postregistration experience, skilled in supporting individuals with serious mental illness, confident in using IT systems, and able to work autonomously within a primary care setting while providing specialist mental health knowledge. They will demonstrate evidencebased, innovative clinical practice, hold a full driving licence with access to their own vehicle, and be willing to undertake phlebotomy training as part of the role.
