ACommunity Nursing Associate (Band 4)working with a diabetes focus supports the delivery of high‑quality, holistic nursing care to people living with diabetes within community settings such as primary care networks (PCNs), outpatient clinics, patients’ homes and care homes.
• To provideholistic, person‑centred nursing care that addresses physical, psychological, social, and public health needs.
• To supportdiabetes management and prevention, ensuring patients receive appropriate monitoring, education, and access to essential diabetes care. To work as part of a multi‑disciplinary community team, contributing to health promotion, early intervention, and ongoing support for long‑term conditions
• Monitor patients with diabetes, ensuring access to diabetes care essentials (e.g., blood glucose monitoring, CGM, injectable therapy,foot checks etc).
• Provide lifestyle and self‑management education, promoting diabetes prevention and health improvement.
• Conduct home visits to support patients with diabetes when required

• Provide holistic, person‑centred nursing care to patients in their own homes or clinic settings.
• Work within Nursing Associate parameters of practice, ensuring care aligns with the NHS “6 Cs”. Support Diabetes Management & Prevention
• Deliver care focused on people with diabetes to ensuring they are appropriately monitored and receive the diabetes care essentials.
• Participate in clinics and services that promote early intervention, self‑management, and healthy lifestyle behaviours. Work Under Delegation of a Registered Nurse
• Carry out delegated clinical and care tasks (e.g., observations, wound care, specimen collection) to an agreed standard and competency.
• Follow individualised care plans and escalate concerns appropriately. Manage and Prioritise a Delegated Caseload
• Proactively organise and prioritise patient appointments and home visits within your competency.
• Make autonomous decisions about patient care within agreed boundaries, seeking support when needed. Contribute to Multi‑Disciplinary Team Working
• Collaborate with diabetes specialist nurses, community nurses, PCN colleagues, GPs, and allied health professionals.
• Provide health education, screenings, and advice aimed at diabetes prevention and improving long‑term condition management.
• Promote independence and self‑management among patients. Record Keeping & Administrative Duties
• Maintain comprehensive, accurate, contemporaneous electronic and written patient records.
